New Patients
Please download, print and fill out the required NEW PATIENT FORMS (below) and bring them to your visit in San Fernando medical clinic. Answer all applicable questions to the best of your knowledge and arrive at least 10 min prior to your office visit in order to complete the registration process without running into your scheduled appointment time. This information will help while awaiting your medical records.
If you are a new patient, please arrive at least 30 minutes early in order to complete the registration process.
Please bring your insurance card to each visit. It is helpful if you know your office visit co-pay.
Please bring all your medical records and any imaging studies that you may have as well as any medications, herbals, vitamins and supplements your are taking.
Please bring payment in the form of cash or credit card (Visa, Master card, Discover, and American Express)
